[Chinese expert consensus on the management of immune-related adverse events associated with perioperative immunotherapy for gastrointestinal tumors (2026 edition)].

Résumé

In recent years,immunotherapy has made breakthrough progress in the perioperative treatment of gastrointestinal tumors. Immunotherapy could improve pathologic response rates and long-term survival,but it can also cause a series of immune-related adverse events (irAEs). irAEs often involve multiple organs or systems,and severe irAEs may cause a potential fatal risk. Moreover,irAEs can be easily confused with perioperative complications,leading to diagnostic difficulties and treatment delays. Currently,there are no clinical practice guidelines or expert consensuses specifically addressing the adverse reactions of perioperative immunotherapy for gastrointestinal tumors,resulting in a lack of clear standards for clinical application. To better guide Chinese clinicians in the management of irAEs during the perioperative period of gastrointestinal tumors,the Committee of Surgical Safety and Quality Control of the China Anti-Cancer Association has organized the development of this consensus - Expert consensus on the management of immune-related adverse events associated with perioperative immunotherapy for gastrointestinal tumors. This consensus covers different scenarios including neoadjuvant/ conversion therapy,intraoperative procedures,and postoperative adjuvant and conversion therapy for gastrointestinal tumors,reaching a consensus on irAE risk management,providing standardized guidance for the clinical diagnosis and treatment of irAEs associated with ICIs in the perioperative period of gastrointestinal tumor patients.
